If you have parental responsibility for a child, you will automatically be the child representative. You won’t need to apply.

If you don’t have parental responsibility for a child and want to become a child representative you need to contact us . We’ll send you a form to complete. A form must be completed for each person applying to become a child representative. This form will ask you to provide information that’ll help us decide if it’s the right thing to make you the child representative instead of the current child representatives.

We’ll also ask you to give us proof of who you are. The Proof of Identity Factsheet shows what information we need to verify your identity.

When you return the completed form we’ll contact you to ask you some questions and to get some more information. There are lots of things we need to think about before we decide whether to make you a child representative.
